We offer free services to people with a range of incurable illnesses including heart failure and dementia – not just cancer. And we support their families, carers and loved ones.
We also support healthcare professionals, including doctors, nurses, and GPs.
Our support includes help with Quality and Outcomes Framework (QOF) points and Gold Standard Framework (GSF) registers.
We’re more than end-of-life care. Our palliative care experts help from day one of a diagnosis. If we help people earlier on it could help improve their quality of life, in the time they have left.
Patients receiving our support are less likely to enter crisis. This reduces the need for urgent or emergency care.
