We offer free services to patients and their families. Additionally, we support healthcare professionals, including doctors, nurses, and GPs.
Our support includes help with Quality and Outcomes Framework (QOF) points and Gold Standard Framework (GSF) registers. We also assist in managing patient crises, which reduces unnecessary calls to GPs, 111, and 999.
Patients receiving our support are less likely to enter crisis. This reduces the need for urgent or emergency care.
We support people with a range of incurable illnesses including heart failure and dementia – not just cancer.
